Questionnaire - Patient Feedback Form

As a small number of patients we have formed a Patient Participation Group (PPG) for Musters Medical Practice. We meet regularly with staff from the practice and want to provide a link between staff and patients. This form will help us find out what you think about the practice, which will enable us to share your views with staff and discuss any issues raised.

Any correspondence addressed to the Patient Participation Group will be opened at PPG meetings. We will do our best to address any issues raised, however please note that our meetings are bi-monthly so if you require a response there may be a delay.
