The practice is registered and complies with the Data Protection Act 2018 (DPA 2018). Any request for access to notes by a patient, patient’s representative or outside body will be dealt with in accordance with the Act.
Such requests should be submitted via a signed request by the patient, for the attention of the Practice Medical Secretaries. These requests may be subject to an administration charge. No information will be released without patient consent unless we are legally obliged to do so.
