Social Prescribing

Many people visit their GP because they feel stressed at work, have financial worries, experience loneliness and isolation, or are grieving the loss of a loved one. These challenges can significantly affect both physical health and mental well-being. However, they cannot always be resolved with medication or a medical approach alone.

Social prescribing takes a holistic approach, considering your overall well-being—physical, emotional, social, and even spiritual. It helps address the root causes of difficulties by connecting you with support for housing, finances, employment, and community involvement.

A social prescribing link worker is part of your General Practice team. Your GP, nurse, or another healthcare professional can refer you to them. They focus on what matters most to you and help you explore what a fulfilling life looks like in your unique situation.

They can connect you with local community groups, voluntary services, and other support networks. They may also provide guidance and information, using their knowledge of available local resources to help you find the right support.

Community Hub

The Community Hub is run by the Rushcliffe Social Prescribing Team and is open most Mondays for people registered with a Rushcliffe GP.​
